Dear IS 349 Family,
Hope all is well. We have rescheduled the NY Smile Free Dental Screening for students. The new date is Thursday, November 2, 2023. The free dental screening provides preventive and restorative care. See above parent letter describing How the Program works and flyer for more information.
If you wish to have your child receive a free dental screening, we urge you to complete a consent form for your child and send it to the school as soon as possible. Parent consent forms may also be completed online using the link below.
